1. For transfer of property,  correction of area, name, lot number etc,  

2. Proof of Payment  - Official Receipt of the current year or Certificate of Tax payment

3. Request form  or Letter of request

4. Special Power of Attorney (SPA) 

5. Valid Identification card - declared owner and authorized representative

   • In case of deceased declared owner - death certificate of the owner (photo copy) & Birth certificate of the heir/s (photo copy)

     • In the absence of the heir/s, special power of attorney with valid id of the heir/s and authorized representative  

   • In case of extrajudicial settlement, present the proof of relationship to the property owner e.g. death certificate, birth certificate, marriage contract 

• Client's request for transfer of property

• Client's request for correction of name, area , lot number  or inscription of name

On site field verification and actual inspection
