Proof of Payment - Official Receipt of the current year or Certificate of Tax payment
Request form or Letter of request
Special Power of Attorney (SPA)
Valid Identification card - declared owner and authorized representative
In case of deceased declared owner - death certificate of the owner (photo copy) & Birth certificate of the heir/s (photo copy)
In the absence of the heir/s, special power of attorney with valid id of the heir/s and authorized representative
In case of extrajudicial settlement, present the proof of relationship to the property owner e.g. death certificate, birth certificate, marriage contract
